Bowl Donation Pledge Form 

Input from our bowl donors helps us to figure out how many tickets we should sell and how much space we'll need to display the bowls and artwork.  Thank you for completing this pledge form!

How many soup bowls are you planning to donate? What material are the bowls? Of course, we can always make room for extra bowls, so feel free to exceed your goal!
This would include gallery bowls or other pieces of fine artwork that you plan to donate. Gallery bowls are high-end pieces that are individually priced for sale. PLEASE INCLUDE: Name of item(s) & Description (title, medium, size, etc. )
Note the date of delivery to St. John’s United Methodist Church (1207 Peabody Avenue Memphis, TN 38104). OR Note the pick-up date and the address and contact for the pick-up location. We'd greatly appreciate all bowl donations on or before Friday, November 9, 2018.
